New Student Enrollment Form

Please fill out one form for each student enrolling. An annual registration fee is due at the time of initial enrollment and on August 1st of each successive year. Registration Fee is due at time of enrollment and is non-refundable. If a break in enrollment occurs (i.e. you decide to leave the studio for one of more months), then the registration fee is due upon re-enrollment, even if the registration fee has already been paid for that enrollment year.

Check

You may turn in your check payment to your instructor. To ensure that your payment is accurately recorded, please include on the check the student's name, class, and month or quarter you are paying for.

Cash

You may turn in your cash payment to your instructor. If paying with cash, please enclose the payment in an envelope with the student's name, class, and month or quarter you are paying for.
