     Reserve your spot today by using Pay Pal to pay for your registration fee, tuition, and uniform fee (pink, white, lilac, blue and peach classes only). Please indicate in the instructions, the name of the student and the class you are enrolling in. If purchased, bring your receipt to class to receive your uniform;

    You may also pay your registration fee, tuition, and uniform fee at your first class, however, space will not be reserved until your registration fee and tuition are received.

Studio Policies: August 2009 to July 2010

Quarterly System

Tuition is based on a quarterly system. Receipt of an enrollment, assumes enrollment for an entire quarter. The quarters begin as follows:

1^st QUARTER: August 1^st

2^nd QUARTER: November 1^st

3^rd QUARTER: February 1^st

4^th QUARTER: May 1^st

Classes regularly scheduled on days when the studio is closed may be made up during alternate class times during the same month.

REGISTRATION FEE

An annual registration fee is due at the time of initial enrollment and on August 1^st of each successive year.

For students enrolled prior to August 1^st, 2009, the registration fee is due yearly on August 1^st. The registration fees are as follows:

$30.00: Single Student Enrollment

$50.00: Family Enrollment.

TUITION

Monthly Installments

Payments for a quarter may be made in monthly installments and are due on the 1^st of each month. Monthly installments not received by the 15^th of the month are considered late. The receipt of an enrollment assumes enrollment for an entire quarter, and that all monthly installments within the quarter must be paid.

Withdrawal

If you withdraw from the studio mid-quarter, a written notice of withdrawal must be received 30 days prior to the withdrawal.

Lump Sum Quarterly Payment

Payment for a quarter may also be made in one lump sum at a discounted rate. Lump sum payments for the quarter are due on August 1^st, November 1^st, February 1^st, and May 1^st. Quarterly payments not received by the 15^th of the month in which they are due, are considered late, the quarterly discount will no longer apply, and you will be billed at the monthly rate.

Tuition is Non-Refundable

Students who withdraw, do not attend class, are on vacation, ill, or are otherwise absent, will not receive a cash refund or credit.

Making up Missed Classes

Classes missed may be made up within the quarter during other scheduled class times, with teacher approval.

Late Payments & Returned Check Fees

A late fee of $15.00 is charged for all tuition paid after the 15^th of the month, and on the first day of each month thereafter. In addition to the late payment fee, returned checks will be assessed a $25.00 returned check fee.

UNIFORM FEE

For new students enrolling in the Pink, White, Lilac, Blue, and Peach classes, a uniform must be purchased. The uniform includes a leotard in the class color, a skirt in the class color, white socks, a hair ribbon in the class color, and pink ballet shoes. After the purchase of the initial uniform, individual pieces may be purchased.

CLASSROOM POLICIES

• Student observation week is the last week of each month.

• Proper dance education requires that the teacher touch the student during class to correct placement and movement when needed.

• Pamela Hayes Classical Ballet will not be responsible for illness or injury.
